Food allegy has been reported as a possible cause of hyperkinetic syndrome, migraine and epilepsy, which was documented with oligoantigenic diet trial. We experienced one case of food allerygy induced epilepsy with hyperkinesia in a 5-year-old
boy.
He
had fed only rice-based food due to milk allerygy. Since normal regular diet was started at 18-month of his age, he had suffered from generalized tonic-clonic seizures, occurred after chocolate, milk-associated food such as yogurt ingestion. The
epilepsy poorly responded to anticonvulsant with 3-4 times per year. And he showed hyperkinetic behavior since 1 year ago. On laboratory studies, total eosinophil count and percentage of eosinophil increased but other immunolgic tests including
IgE
RAST, IgE PRIST were normal. EEG showed normal sleep pattern and Brain MRI also revealed no abnormal parenchymal lesion. Attention deficit hyperactive disorder was suspected on psychiatric evaluation. He was given an oligoantigenic diet,
consiting
of
two meats(beef, fish), two carbohydrate(rice, potato), vegetables, water, a few spices, calcium. During 3 week of diet therapy, no seizure occurred and his hyperkinetic behavior much improved on Coner Scale and Psychometry follow-up and total
eosinophil
count also much decreased. He responded to diet therapy and he entered reintroduction phase for identification of provoking foods. After milk was reintroduced 3 day later, hyperkinetic behavior was aggravated. The seizure attack reproduced during
subsequential reintroduction of chocolate. The intervals of seizure to react chocolate, milk were 2 days, 7 days respectively. To our knowledge, this is the fist report in Korea. So we report this case and brief literatures of food allergy
induced
epilepsy with attention deficit hyperactive disorder.
